In plain terms

For individuals with prior gout attacks, consuming high-purine meals over a two-day period raises the odds of an acute recurrent gout attack nearly 5-fold compared to low-purine days, even while taking uric-acid-lowering medications. Consuming two to four alcoholic drinks in the preceding 24 hours raises attack risk by ~50%, across beer, liquor, and wine alike.

Cost: what it takes

Zero financial outlay. Simply order fewer skewers of meat and seafood dishes, and minimize alcoholic beverages during social dinners. The difficulty is exerting dietary discipline at social dining tables.

Benefit: what the source reports

A 1-year prospective internet-based case-crossover study of 633 gout patients compared dietary intake during the 2-day period preceding an acute gout flare with flare-free control periods in the same individuals. Purines are natural chemical compounds in food metabolized into serum uric acid. Dividing total 2-day purine intake into quintiles revealed that compared with the lowest quintile, the subsequent four quintiles showed odds ratios of recurrent gout attacks of 1.17, 1.38, 2.21, and 4.76 respectively (P for trend < 0.001). For animal purines alone, odds ratios were 1.42, 1.34, 1.77, and 2.41. For plant-derived purines, odds ratios were 1.12, 0.99, 1.32, and 1.39. Subgroup analyses stratified by sex, alcohol consumption, diuretic use, allopurinol therapy, NSAID use, and colchicine prophylaxis yielded consistent findings. A companion study of 724 gout patients evaluated alcohol intake within 24 hours prior to flares: compared with zero alcohol, 1 to 2 drinks increased recurrence risk by 36% (OR 1.36, 95% CI: 1.00–1.88) and 2 to 4 drinks increased risk by 51% (OR 1.51, 95% CI: 1.09–2.09), with risk increasing across wine, beer, and distilled spirits. In incident gout, a 12-year prospective cohort study of 47,150 male health professionals demonstrated that the highest quintile of meat consumption had a 41% higher risk of developing gout than the lowest quintile (RR 1.41, 95% CI: 1.07–1.86), and seafood had a 51% higher risk (RR 1.51, 95% CI: 1.17–1.95), while high dairy consumption reduced risk by 44% (RR 0.56, 95% CI: 0.42–0.74). In the same cohort, each additional 12 oz (355 mL) can of beer per day increased incident gout risk by 49% (RR 1.49, 95% CI: 1.32–1.70), and spirits increased risk by 15% per drink daily (RR 1.15, 95% CI: 1.04–1.28).

Conditions, limits and disagreements

Directed primarily at individuals with established gout flares. In individuals without gout, dietary purine intake has a much smaller impact on baseline serum uric acid than commonly believed: in 16,760 white adults without gout, diet explained less than 0.3% of variance in serum urate, whereas common genetic variants explained 23.9%. Thus, while high-purine dining triggers acute attacks, underlying genetic predisposition dominates disease onset. These studies evaluated total purine and ethanol intake rather than cooking technique (cold seafood with beer triggers identical risks to grilled skewers). Dietary restraint cannot replace uric-acid-lowering pharmacotherapy (see Chapter 16, Rule 9). Reducing processed meat and alcohol for cardiovascular mortality is detailed in Chapter 2, Rules 18 and 19.
