In plain terms
Only about 30% of inhaler users execute perfect technique; critical errors prevent the aerosol drug from reaching deep airways. Patients making critical errors suffer twice the rate of severe exacerbations and emergency hospitalizations. Furthermore, participating in pulmonary rehabilitation following hospitalization for an acute exacerbation cuts subsequent rehospitalization by more than 50% while improving exercise capacity and quality of life.

Cost: what it takes
Demonstrating inhaler technique during regular clinical follow-ups incurs zero extra fee beyond regular visits, taking just a few minutes with your device. Supervised pulmonary rehabilitation involves structured exercise sessions, medication coaching, and self-management education across multiple clinical visits, with costs varying by hospital tier. The challenge is persevering with physical training when recovering from a debilitating hospital stay.
Benefit: what the source reports
The China Pulmonary Health (CPH) national survey of 50,991 adults aged >=20 undergoing spirometry showed an overall COPD prevalence of 8.6% (99.9 million individuals), rising to 13.7% among adults aged >=40; only 12.0% of COPD patients had ever received prior spirometry testing. A systematic review of 144 studies evaluating 54,354 patients observed by trained assessors found that only 31% demonstrated correct inhaler technique, 41% showed acceptable technique, and 31% showed poor technique, with zero improvement across two decades. For pressurized metered-dose inhalers (pMDIs), 45% failed to coordinate actuation with inhalation, 44% inhaled too slowly or shallowly, and 46% failed post-inhalation breath-holding. In a real-world French observational study of 2,935 COPD patients, over half made errors; critical device-handling errors that prevent drug delivery occurred in 15.4% to 46.9% across device types. The rate of severe exacerbations requiring hospitalization or ER visits in the preceding 3 months was 3.3% in error-free patients vs. 6.9% in those making critical errors (OR 1.86, 95% CI: 1.14–3.04). The Global Initiative for Chronic Obstructive Lung Disease (GOLD 2023) guidelines mandate educating patients on inhaler mechanics and re-evaluating technique at every follow-up visit. A Cochrane 2016 systematic review of 20 randomized controlled trials (1,477 participants) evaluating pulmonary rehabilitation following an acute COPD exacerbation vs. conventional care found that rehabilitation reduced hospital readmissions by ~56% (OR 0.44, 95% CI: 0.21–0.91; moderate-quality evidence across 8 trials and 810 patients). Six trials evaluating mortality showed OR 0.68 (95% CI: 0.28–1.67; low-quality evidence). Statistically and clinically significant improvements occurred in health-related quality of life and 6-minute walking distance (+62 meters, 95% CI: 38–86; high-quality evidence).
Conditions, limits and disagreements
Controversy: Optimal timing of post-exacerbation rehabilitation is debated. A UK trial initiating intensive exercise within 48 hours of acute inpatient admission showed no reduction in 1-year readmissions (HR 1.1) and higher mortality (OR 1.74, 95% CI: 1.05–2.88). GOLD guidance highlights that commencing rehabilitation within 4 weeks of discharge yields clear benefit, but cautioning against aggressive exertion during acute decompensation. For inhalers, specific delivery mechanics vary across Dry Powder Inhalers (DPIs), Soft Mist Inhalers (SMIs), and pMDIs; follow device instruction leaflets and physician demonstrations. Annual influenza vaccination is covered in Chapter 1, Rule 20; pneumococcal vaccination in Chapter 1, Rule 22; smoking cessation pharmacotherapy in Chapter 2, Rule 3.
