In plain terms
For individuals struggling with insomnia, taking melatonin advances sleep onset by an average of roughly 7 minutes and extends total sleep duration by only about 8 minutes compared to placebo. The American Academy of Sleep Medicine advises against using melatonin to treat chronic insomnia. In China, melatonin is marketed as a health supplement labeled with "this product cannot replace medication." However, melatonin is effective for alleviating jet lag when flying across five or more time zones.

Cost: what it takes
Zero financial cost; saves money spent on ongoing commercial melatonin purchases. The difficulty is convenience: swallowing an over-the-counter gummy or pill is far easier than committing to weeks of clinical behavioral restructuring.
Benefit: what the source reports
A 2013 meta-analysis pooled 19 randomized, placebo-controlled trials involving 1,683 participants with primary sleep disorders (including 14 trials specifically on insomnia): melatonin reduced sleep onset latency by an average of 7.06 minutes (95% CI 4.37–9.75), increased total sleep time by 8.25 minutes (95% CI 1.74–14.75), and produced a negligible improvement in sleep quality (SMD 0.22, 95% CI 0.12–0.32), lagging substantially behind prescription sleep aids. In clinical practice guidelines for chronic insomnia issued by the American Academy of Sleep Medicine (AASM, 2017), the panel issued a "weak" recommendation against clinicians using melatonin for sleep onset or sleep maintenance insomnia, evaluating trials in adults aged >=55 and judging the overall evidence quality as "very low" with pooled sleep quality improvements failing to meet clinical significance thresholds (SMD 0.21, 95% CI -0.36 to 0.77). Health supplements containing melatonin in China carry explicit warnings: "This product is not a substitute for pharmaceutical drugs," and are not suitable for children, adolescents, pregnant women, or nursing mothers. In contrast, for acute circadian shifts, a Cochrane systematic review of 10 randomized trials demonstrated that taking 0.5 to 5 mg of melatonin near local bedtime after flying across five or more time zones significantly reduces jet lag symptoms compared to placebo. Melatonin appears deceptively appealing because it is an endogenous pineal hormone perceived as harmless and available without prescription.
Conditions, limits and disagreements
Controverted in commercial wellness literature. Authors of the 2013 meta-analysis noted that melatonin exhibits minimal tolerance and low adverse events, suggesting a modest adjunctive role. However, trial data in older adults demonstrate that the absolute benefit is simply too small to resolve clinical insomnia disorder. First-line therapy for chronic insomnia is CBT-I, detailed in Chapter 3, Rule 26. Snoring and daytime drowsiness warrant screening for obstructive sleep apnea (Chapter 3, Rule 27). Product labeling notes drivers and machinery operators must not take melatonin immediately prior to work. Benefit magnitude is categorized as Low based on financial savings from unproven supplements. Primary beneficiary is yourself.
