In plain terms

If the child can still cough or cry audibly, let them cough while you watch closely. If they cannot cry, their face turns blue, call 120, lay the baby face-down on your forearm, and give 5 back blows with the heel of your hand between the shoulder blades. If that fails, roll them over and give 5 quick thrusts on the center of the chest. Keep alternating — never push on the belly. If the baby becomes unresponsive, switch to CPR.

Cost: what it takes

No cost. Watch a demonstration once or take a short infant first-aid class — a few minutes to half a day. The hard part is staying calm enough to turn the baby face-down on your forearm.

Benefit: what the source reports

The American Heart Association and American Academy of Pediatrics issued 2025 pediatric basic life support guidance. It states that choking (for example, an object blocking the airway) “is a leading cause of death in infants and children.” Liquids are what infants most often aspirate. Mild obstruction: still coughing or making sounds — let the child cough while you monitor for worsening. Severe obstruction: weak cough or none, cannot cry, cyanosis — call emergency services first, then “repeat cycles of 5 back blows and 5 chest thrusts until the object is expelled or the infant becomes unresponsive.” Abdominal thrusts are not recommended for infants because they can injure internal organs. Chest thrusts are now taught with the heel of one hand because it achieves greater depth than the old two-finger method. When the infant is unresponsive, start CPR with compressions first; do not check a pulse. Remove only visible objects before giving breaths. The guideline explicitly says “do not perform blind finger sweeps.” The International Federation of Red Cross and Red Crescent Societies 2025 international first-aid guidance uses the same sequence: up to 5 firm back blows between the shoulder blades, then turn the infant and give up to 5 chest thrusts, then call emergency services if needed. Its chest-thrust technique uses a two-thumb encircling grip. The Fujian Provincial Red Cross emergency-care training center describes “5 back blows + 5 chest thrusts” for infants under 1 year: hold the infant prone on the forearm with the head lower than the trunk for back blows.

Conditions, limits and disagreements

Rated B because support comes mainly from case reports, with no “how many fewer deaths if you follow this” figure. Benefit is rated large by judgment: a fully blocked airway can stop an infant’s heart within minutes. The main beneficiaries are your own children. For children 1 year and older and adults, use abdominal thrusts; see item 26 in this section (choking: back blows plus abdominal thrusts). Chest-thrust hand position differs across sources: the U.S. guideline and newer Chinese Red Cross training use one-hand heel; IFRC uses two thumbs; older courses used two fingers. The 2025 U.S. guideline dropped two-finger compressions because depth was inadequate. Any of these beats doing nothing. Even after the object is expelled, seek medical care — fragments may remain in the lungs. Keep coins and small toys out of reach. If the baby is unresponsive, see item 44 in this section (infant CPR).
