In plain terms

An intoxicated person who loses consciousness can choke to death on their own vomitus or suffer respiratory arrest. Place them on their side in the recovery position and keep someone stationed beside them continuously; never abandon them to sleep alone in a closed room. Call emergency services immediately if they cannot be awakened, breathe slowly or irregularly (<8 breaths/min or >10-second pauses), seize, or develop cold, clammy, cyanotic skin. Never force coffee, never put them in a cold shower, and never induce vomiting.

Cost: what it takes

Zero monetary cost. Care requires several hours of vigilant physical observation, often lasting through the night. The challenge is remaining alert when you have consumed alcohol yourself, resisting the urge to go home to sleep, and overcoming the pervasive cultural myth that an intoxicated person simply needs to "sleep it off."

Benefit: what the source reports

NHS clinical guidelines list key signs of acute alcohol poisoning: severe mental confusion, slurred speech, loss of coordination, vomiting, incontinence, pale or bluish/clammy skin, slow or irregular respiration, seizures, and unresponsiveness. If alcohol poisoning is suspected, or the individual is unconscious or stops breathing, immediately call emergency services. Core action protocol: remain by their side continuously because they risk suffocating on vomit or experiencing respiratory depression; keep awake individuals seated upright; place unconscious breathing individuals into the lateral recovery position; offer small sips of water only if fully conscious and able to swallow; and keep them warm with coats or blankets. Five critical contraindications: (1) never give more alcohol; (2) never feed food, which induces airway aspiration; (3) never administer coffee or caffeinated drinks, which worsen dehydration; (4) never put them under a cold shower or in a cold bath, which precipitates severe hypothermia, falls, or drowning; and (5) never attempt to induce vomiting. Emergency department clinical data published by Shaanxi Provincial Health Commission highlight that alcohol depresses laryngeal protective reflexes, making vomitus easily aspirated into trachea and bronchial trees, causing asphyxiation and immediate death; emergency teams frequently recover intoxicated individuals already in fatal cardiac arrest packed with pulmonary aspiration debris. In stuporous or comatose stages marked by cyanosis, cold extremities, hypothermia, and shallow breathing, call emergency services immediately, maintain a lateral position without emesis, apply warm blankets, and initiate CPR immediately if breathing ceases. Beijing Municipal Health Commission advises: "Never let an intoxicated individual sleep alone to avoid unmonitored asphyxiation." CDC statistics indicate an average of 2,221 annual alcohol poisoning deaths among individuals aged 15 and older in the United States (roughly 6 deaths per day).

Conditions, limits and disagreements

Graded Evidence Grade B based on official health authority emergency guidelines and clinical consensus. Benefit is categorized as High because asphyxiation and respiratory arrest occur almost exclusively when victims are left unmonitored. Never attempt manual throat-tickling emesis on lethargic or unresponsive individuals. Beyond family members, these duties apply heavily to dining companions and colleagues: under tort law, co-drinkers share affirmative legal duties of reasonable care and safety. In Jiangsu Taixing, co-drinkers who failed to notify family or escort an incapacitated colleague home before he drowned in a river were held liable for 7% and 3% comparative fault (apportioning 103,887 RMB in damages). Supreme People's Court Guiding Case No. 227 affirmed that dining companions failing to fulfill warning, care, and safe escort duties bear comparative fault. Always escort drunk companions directly into the custody of family or remain until sobriety returns. If discovering an unknown intoxicated person collapsed outdoors, call emergency dispatchers without attempting forceful repositioning unless necessary (see Chapter 13, Rule 2). Alcohol impairs thermoregulation; manage hypothermia as outlined in Chapter 13, Rule 28. Alcohol interactions with cephalosporin or metronidazole antibiotics causing disulfiram-like reactions are detailed in Chapter 34, Rule 11. Never take hot baths or enter saunas after drinking (see Chapter 22, Rule 11).
